This has never actually happened to me (mostly because I don't have a long enough attention span to accuse everyone of being a vampire), but it's quite possible to do this. And that's the problem I think. A complete nobody randomly calls someone out as being a vampire, and if it just so happens that they're right, they'll reveal themselves to be a vampire.
Personally, I think the ability to accuse someone of being a vampire should only work if you either have a quest to kill that vampire, or your reputation is higher than their position.

By the way, if you're ever playing Dwarf Fortress and you get given a quest to kill a vampire, but you have no idea where he is. Just go to the town or hamlet he's supposed to be, and ask for a quest from someone else. That person should then point you in the general direction of the vampire, instead of giving you a new quest.
It took me years to learn that.
Oh, and they usually like to spend their time in the keeps, when they're in towns that is.
